Family doctors, or general practitioners (GPs), are your go-to healthcare providers for a wide range of health needs, offering care that covers everything from preventing illness to treating common conditions and managing long-term health issues.

The family practice is now closed and is no longer providing appointments or accepting new patients.
Your original medical records are securely stored with MedRecords, a non-profit medical records organization. Patients are encouraged to transfer a copy of their record to their new healthcare provider or retain a personal copy.

Once the practice closed, Dr. Bax no longer had access to the medical record system and is unable to provide ongoing follow-up, prescription renewals, investigation results, referrals, or other clinical care through Boundless Medical.
If you do not yet have a family doctor, we encourage you to register with the Health Connect Registry, which helps connect people in British Columbia who are without a regular primary care provider with available services.
Boundless Medical remains an umbrella for future healthcare projects and innovation work, but is not currently providing family practice services.
MODERN, SIMPLE, PATIENT-CENTERED
Boundless Medical Family Practice was designed as a simple, flexible solo family practice focused on patient convenience, comfort, and ease of access.
Appointments were booked online, with both in-person and telehealth options available where appropriate. Automated reminders reduced the need for phone calls, waiting on hold, and back-and-forth scheduling.
We also used secure AI-supported tools to assist with clinical documentation, helping appointments remain focused on the patient rather than the screen.
For in-person visits, patients were welcomed with complimentary tea or coffee before being called in personally by the doctor.
As a solo, part-time practice, the model did not rely on a full-time receptionist or traditional telephone booking system. It was designed to give patients greater control, convenience, and a calmer healthcare experience.

The practice was located within Inlet Pharmacy in Port Moody but operated independently from the pharmacy. Inlet Pharmacy staff do not have access to Boundless Medical patient records and cannot assist with appointments, results, referrals, or other clinic-related matters.

Inlet Pharmacy continues to provide its usual pharmacy services and may also be able to assist with a range of minor ailments, vaccinations, and other pharmacy-based care.
